Information is critical to
developing patient focussed relationships
between general practitioners and hospital clinicians. At
the initial work-stream meetings, general practitioners identified access
to information as the key tool to enable them to care for their patients
more effectively. The Canterbury Initiative approached a company already
publishing clinical information for the Canterbury
and discussed the challenges of informing general practice with minimal
barriers to access. The result is HealthPathways – a website two clicks away
from their patient management systems.
HealthPathways provides a range of information jointly developed by
small groups of hospital and general practice clinicians. The
information is initially made available on a draft website, where it is
reviewed by a wider group of stakeholders before being made available to
all of Canterbury
practice – from Kaikoura to Ashburton.
Access to HealthPathways is restricted to health professionals
responsible for referring to specialty services.
